Premium Video Package

• Edited Game Footage (15 minutes max)

• Customer to supply game footage

• Package includes 5 DVD's in flexible plastic cases

• Picture on front, schedule or stats on back of DVD cover

• Player resume (insert)

• Additional DVD's are $5.00 each

Total Cost:  $166.50 (includes sales tax)  Please make checks payable to:  CONCOR, LLC

Travel Fee:  May be applicable depending on location of video shoot

Credit Cards:  Now accepting Credits Cards per Customer requests.  A $3.00 charge is applicable for this service.

Shipping Charge:  May apply if customer is not able to pick up finished package in Dearborn

Insufficient Funds Charge:  A $20.00 charge will be applied for any returned checks
